The Northern Kentucky Xtreme was founded in 2007 by Jack and Shannon Patterson under the Xtreme Sports Mgmt LLC the team then split into two teams with the expansion Tri-State Sharks after the 2007 season. The team was then handed over to the Mike Kirchgessner and Kevin Hess who brought in long time semi-pro coach Tony Bray. In 2008 the Southern Ohio Patriots merged with the renamed Kentucky/Ohio Xtreme and moved from Highland Heights High School across the river to Colerain High School. In 2009 the Xtreme made the move to the United States Football Alliance and will also be playing their games at Newport High School.
